Each appointment will be charged through credit or debit card at end of our session. Please contact us to discuss pricing.
As a courtesy, we will print off a superbill at the end of the visit, which can subsequently be sent to your insurance company seeking reimbursement. This does not guarantee reimbursement, and patient/guardian will be responsible for all charges.

- We will bill your insurance directly but are out of network for the following:
- DMBA
- Discounts are given for prompt payment when paying cash for visits. Please contact our office for details.
Pay Your Bill
Bills can be paid in on of the the following ways:
- Mailing a check written to “SMILE Psychiatry” to: 138 E 12300 S, Unit 328, Draper, UT 84020
- Calling 385-442-6112 and providing payment over the phone
- Logging into your patient portal account – see button below
